New patient registration
HOW TO REGISTER
To Register as a patient with us, we will need you to complete some registration documents. You can do this online, collect one directly from us or download one below.
We highly recommend registering with us online as it is a convenient, free and secure service. There is less chance of duplication and mistakes being made and it reduces administrative processing time and improves access to care services.
- ONLINE - download the NHS app or fill in the form on the NHS website
- DOWNLOAD/ COLLECTION - If you have limited or no access to online services, please download a copy here or collect one from the surgery.
Registration is with the practice and not an individual doctor, although you will be allocated a named GP.
You do not need to show ID, proof of address or immigration status to register as a patient.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Please make sure you have enough medication to cover you during this process by asking your current practice to supply you with one month of medication.

Please check your answers carefully and ensure you have completed all the questions. Missing information or incorrect information slows down the registration process as our staff will need to contact you to gain the correct details.
Once we have received the fully completed and signed registration form, your registration will be processed within 2 weeks by the Registrations Team.
Please contact the surgery after 2 weeks to confirm registration.
As soon as you are registered as a patient with Richmond Hill Practice Ltd, you can then make any appointments you require.
OUT OF AREA
You can register as an out-of-area patient and will be entitled to the full range of care, except home visits.
TEMPORARY PATIENT
If you are staying in the area temporarily or visiting and are unwell, please contact either 111 or call our surgery telephone number 01282 911401, as we may be able to treat you as a temporary patient.
