ALL APPOINTMENT REQUESTS (high priority and routine) to see a GP will be triaged, and you will need to fill in a simple e-consultation form.

For all high priority requests, please contact the surgery between 8am - 10am. After this time, if your request is high priority you will need to call us on 01282 911401. Routine Nurse appointments can be booked on the telephone as usual.

ROUTINE ADMIN- click on submit a request or click download NHS app - fitnotes, test results, admin, medication, self-referral information, routine health concerns and routine medical issues.
